Illinois State Opioid Survey

we are participating in a number of State of Illinois councils and committees that are actively working on solutions to the current opioid epidemic. To assist in solving the crisis, the Illinois Department of Human Services/Division of Alcoholism and Substance Abuse Services (IDHS/DASA) and Advocates for Human Potential, Inc. (AHP, a consulting contractor working with DHS) have created a short, anonymous web survey that asks people about activities taking place in their communities that are addressing the opioid epidemic and what they think the State needs to do to solve it.
