What parts of the NBCE examination does the State of Illinois require?

What parts of the NBCE examination does the State of Illinois require for licensure as a chiropractic physician?

For all applications filed on or before June 30, 2016, Illinois requires successful completion of the NBCE Parts I, II and III.  For all applications filed on or after July 1, 2016, Illinois requires successful completion of Parts I, II, III and IV.
