Some of our members have asked for a sample disclaimer that they may use when providing non-therapeutic or non-medical services to patients. Although it may be helpful to provide a disclaimer, it does not relieve you of the obligation to comply with laws regarding consistent fee schedules, laws on inducements, or terms of provider contracts, so be mindful when structuring your services.
"I understand and agree that the aforementioned services are being performed at my request and are not medical in nature. These services are non-therapeutic and are not part of a diagnosis or treatment plan. I understand and agree that Dr.________________D.C. will not submit these services to my health plan if any and that I will be financially responsible for payment."
